This page explains how your protected health information (PHI) may be used and shared as a client of Lumara Integrative Health, and outlines your rights under the Health Insurance Portability and Accountability Act (HIPAA). It also shares the ways Lumara Integrative Health is committed to protecting your privacy with care, clarity, and integrity.

1. Your Health Information Rights As a client, you have rights regarding how your health information is used and accessed. These include:

  • Access to Records. You may request a copy of your health records at any time. Please note that psychotherapy process notes (private reflections used solely by your therapist) are not typically included in these requests unless otherwise discussed.
  • Requesting Corrections. If you believe something in your record is incomplete or inaccurate, you may ask for a correction.
  • Confidential Communication Preferences. You can specify how and where you’d like to be contacted (e.g., email only, phone without identifying information).
  • Requesting Limitations. You may request limits on what we share. While we will do our best to honor those requests, there are certain situations where we may be required to share information (e.g., safety concerns or legal obligations).
  • Disclosure Records. You have the right to know when and why your PHI has been shared with others.
  • Copy of This Notice. You may request a printed or digital version of this Notice at any time.

2. How Your Information May Be Used Your PHI may be used for your care, including coordinating care across providers with your permission (e.g., sharing information with a physician, psychiatrist, or functional medicine practitioner you work with). It may also be used for practice operations such as scheduling, billing, internal review, and quality assurance in the delivery of ethical care. When legally required, your information may be used to comply with laws related to mandated reporting, legal orders, or situations involving potential harm.

Your information will never be sold or used for marketing purposes without your written consent.

3. When Information May Be Shared Without Your Written Consent While your privacy is deeply respected, certain situations require or allow the release of information, even without written authorization. These include:

  • Clear and serious threats to your safety or the safety of others
  • Mandatory reporting of abuse or neglect (e.g., child, elder, dependent adult)
  • Valid court orders, subpoenas, or legal investigations
  • Public health requirements (e.g., communicable diseases)
  • Government audits or compliance checks

In all cases, only the minimum necessary information will be shared, and we will inform you when possible.

4. Your Consent and Choice In most other circumstances, your PHI will only be shared if you’ve signed a written authorization. You may revoke this consent at any time in writing, and we will honor it moving forward.

5. Our Responsibilities Lumara Integrative Health is legally and ethically committed to protecting the privacy and security of your health information, notifying you if a data breach compromises your PHI, providing you with this Notice and keeping it up to date, and following the terms outlined in this document.

6. Changes to This Notice Any updates to this Notice will be posted on our website and available upon request. Changes will be reflected by an updated effective date at the top of this page.
